Motivated by business pressures, market consolidation, and the pursuit of quality care, health care professionals in all areas of practice are beginning to explore more fully the tremendous potential of disease management.

Recommended by the American Hospital Association (AHA), this best-selling book lays out the system-wide strategy for proactively managing chronic diseases across the entire continuum of care.

Disease Management is the first book to bring together systems thinking and organizational structure in a framework for designing, developing, and implementing a comprehensive health management system.

"A well-reasoned timely book which will inform the larger health care discussion as we move into the 21st century." — from the foreword by Senator Bill First

Warren E. Todd is actively involved in disease management and helped engineer an asthma disease management program at the National Jewish Center for Immunology and Respiratory Medicine in Denver. Currently, he serves as vice-president of business development in Hasting Healthcare Group in Pennington, New Jersey.

David B. Nash, MD, nationally recognized for his work in outcomes management, medical staff development, and quality care issues, is the first director of the Office of Health Policy and Clinical Outcomes at Thomas Jefferson University and associate professor of medicine at Jefferson Medical College in Philadelphia.

The American Hospital Association (AHA) is the national organization that represents and serves all types of hospitals, health care networks, and their patients and communities. Close to 5,000 hospitals, health care systems, networks, other providers of care and 37,000 individual members come together to form the AHA. Founded in 1898, the AHA provides education for health care leaders and is a source of information on health care issues and trends. The Health Forum was created in the fall of 1998 through the union of The Healthcare Forum and the American Hospital Association's publishing and data and information subsidiaries.
