Written by an expert who has practiced and taught the practical side and successful use of the E/M coding system since its inception, this book provides proven solutions to change E/M coding into an invaluable asset.

Physicians, medical students, administrators, coding professionals, chart auditors, and others will discover this book to be a solid learning tool and guide to completing E/M documentation and coding correctly and in a timely fashion while enhancing patient care.

  • This approach is built on five medical record principles that differ fundamentally from the traditional E/M method - Practical E/M provides essential tools for creating a workable medical record that incorporates the CPT coding guidelines and the Documentation Guidelines for E/M Services. This book is built around the CPT E/M documentation principle of the "nature of presenting problem" (NPP) which indicates the level of care and coding warranted by the patient's illness.

  • This approach integrates documentation and coding into the process of providing high-quality care. Learn how to perform and document detailed and comprehensive levels of care that result in E/M compliance for every patient visit.

  • This approach increase productivity - Appropriately increase bottom line revenue by eliminating undercoding and providing the level of documented care and coding warranted by the patient's condition.

The new edition also responds to requests for greater breadth of Intelligent Medical Records (IMR) tools and includes:

  • A Comprehensive set of sample IMR forms for the most common E/M types of services on a CD-ROM
  • Tools for ER departments, nursing and home health care providers that rely on the three components of NPP
  • Tools to assist physicians with other caretakers such as teaching hospitals, residents, nurse practitioners and physician's assistants
  • Important information on receiving American Academy of Professional Coders (AAPC) CEUs: In addition to submitting the required documentation to the American Academy of Professional Coders to obtain the approved CEUs, you must submit the official "certificate of approval" form, provided below. Individuals who do not include this "certificate of approval" run the risk of not receiving full CE credit.

Recommended by the American Medical Association, this is a one-of-a-kind resource that includes commentary sections, helpful tips, and examples that convey the "real world" applications of Practical E/M concepts. This book assists the user in correctly identifying the types of services and the levels of medical necessity.

Excerpts: Acknowledgements, Table of Contents, Introduction, Preface, Prologue

As an added benefit, this resource includes a complete sample intelligent medical record (IMR) form. Use this form as a reference as you work through each IMR level of detail outlined throughout the book.

The American Medical Association helps doctors help patients by uniting physicians nationwide to work on the most important professional and public health issues. The AMA is a major leader in medical publishing, offering resources such as this one that physicians really need and use.

Please remember: The government no longer allows a grace period for annual code sets. The new HIPAA Transaction and Code Set Rule requires providers to use national medical code sets that are valid at the time that a service is provided. ICD-9-CM code revisions become effective October 1st each year while CPT and HCPCS code revisions become effective January 1st. In order for you to meet this requirement, you must have the revised CPT, HCPCS, and ICD-9-CM codes in your possession before the implementation dates. The best way to do this is to order your code books early. Review current coding resources.
