During your next survey, the JCAHO will examine your medication policies and procedures with a fine-toothed comb. 5 of the top 10 most problematic standards relate to medication management.

Be prepared! Due to the sheer volume of medications dispensed each year - thousands to millions of doses, depending upon the size of your facility - medication management is a highly visible patient safety concern. During your next scheduled, or unannounced, survey, the JCAHO wants evidence that you're in compliance with:

  • the current medication management standards,
  • new elements of performance,
  • revamped National Patient Safety Goals,
  • U.S. Pharmacopeia Chapter 797 on compounding sterile preparations,
  • and more.

The Compliance Guide to the JCAHO's Medication Management Standards is your solution to this major non-compliance area. Now in its second edition, this guide has been updated to reflect the JCAHO standards, elements of performance, and the new National Patient Safety goals affecting pharmacy and the medication management process.

You will get forms and policies. Don't develop medication policies and procedures from scratch - use these field-tested forms to prove compliance. This easy-to-read, "how-to" compliance guide in a handy, loose-leaf binder format provides field-tested policies, procedures, and forms you need to prove your compliance with the JCAHO standards and successfully reduce medication errors in your facility. Make sure your medication policies and processes stand up to the JCAHO's scrutiny

This manual provides you with step-by-step tools and advice on exactly how to comply with the latest medication management requirements.

  • More than 45 sample policies, procedures, and forms that you can modify to fit your facility along with quick tips on how to use them efficiently
  • Plain-English explanations and the latest details of the 2005 medication management standards and elements of performance
  • Descriptions of the National Patient Safety Goals, along with insider information on what surveyors look for during survey, and specific forms for proving compliance with the goals
  • Real-life case studies to show you practical examples of compliance. Learn how one hospital took steps to reduce errors caused by look-alike/sound-alike drugs and how others dealt with the recent Vioxx recall
  • Tools to help you organize your compliance efforts, including the medication management preparation for JCAHO survey checklist
Clarifications on which competencies your staff needs and best practices on how to ensure they have the adequate credentials
  • Best-practice strategies to ensure that your facility's compliance efforts are ongoing rather than just episodic

Here's an inside look at just a handful of the policies, procedures, and forms you'll receive (remember, you get more than 45 in all - plus tools and advice to implement immediately in your facility):

  • Pharmacy Emergency Access Form
  • Falls risk assessment communication form
  • Sample medication reconciliation form
  • Sample titration policy
  • USP Chapter 797 gap analysis assessment report
  • Narcotic discrepancy reporting form
  • Sample Card Attachment for Employee Badges
  • Drug Shortages Policy and Procedure
  • Log for Laminar Flow Hood/Biological Safety Cabinet Cleaning
  • Intensive Review of Medication Errors Form
  • High-risk medications policy and procedure
  • National Institute for Occupational Safety and Health Form
  • Timetable for completing various sections of USP 797
  • Self-administration medication program participation policy

  • plus many more
